Jennifer Ellsworth earned her Bachelor’s degree in Social Services from Quinnipiac University in 2008 and her Master’s degree in Social Work from SUNY Albany in 2012. She is a Licensed Clinical Social Worker in the State of New Hampshire.
Jennifer brings over 16 years of experience in the healthcare field, with a background that includes both direct clinical practice and project management. She has worked across a range of settings, including hospitals, clinics, hospice care, and community-based social work, with a particular focus on complex care management and transitions of care.
Outside of her professional work, Jennifer enjoys spending time with her husband and son, traveling, hiking New Hampshire’s mountains, and crafting.
