"Help at Home" Transitional Housing with Intensive Supports
FELLOWSHIP
HOUSE People in need of 24-hour/day support are admitted to Fellowship House following an in-depth assessment of their abilities, needs and interest. Short and long-term residential goals are established with each resident in concert with the person’s treatment plan from the mental health center. Program activities are designed to assist the individual’s recovery by providing psychosocial interventions and program staff work collaboratively with the person’s natural support systems - other community providers, family and friends. Fellowship House residents receive assistance with building their skills for everyday living and with managing the symptoms of their illness. Residents are expected to adhere to any physician prescribed routines, i.e., taking medications, and the staff assist the person in maintaining a schedule. The residents are involved in making decisions about the household and in activities in the community to help make the transition to greater independence. They are responsible for the upkeep of their private room, the common areas and meal preparation and for actively working toward their goals. The expected length of stay is between six and twenty-four months. The Fellowship House facility is a twelve-bedroom, three-story building. Residents have private rooms and share bathrooms, kitchen, dining area and living room. Smoking is not permitted indoors. The building is located within a couple of blocks of Concord’s downtown area and other services including public transportation. When a resident is ready to move on, Fellowship Housing staff can help with finding suitable housing, often in one of our own projects. The departing person may experience a need to stay connected to the group home and receive staff support. “Graduates” are welcomed back for meals, conversations, special occasions and group activities. Please refer to the Outreach Services section for a description of after care assistance. Fellowship House is a Housing and Urban Development (HUD) Demonstration 202/8 Project which opened in 1984. It is licensed and certified by the NH Department of Health and Human Services as a community residence.
