Outreach Services Fellowship Housing contracts with Riverbend Community Mental Health, Inc. (Riverbend) to provide Outreach Services to designated individuals living in the community. Annually, Fellowship Housing and Riverbend negotiate the number of units of service to be provided and referrals are accepted throughout the year. The overall purpose of these clinical, medically necessary services is to help the individual build and maintain the skills and supports necessary to achieve their goals despite having a mental illness. Fellowship Housing staff provide rehabilitative services in natural community environments which helps to reinforce and enhance the person’s ability to successfully manage their symptoms. These services often have the effect of helping the person avoid hospitalization, and can also increase one’s confidence and progress toward their recovery. Outreach Services relate to the goals and objectives on the client’s Individual Service Plan, which they develop with their Riverbend treatment team. The services are face-to-face interactions in which the service provider uses their clinical judgment and considers his/her knowledge of the client, the manifestations of the client’s mental illness, rehabilitation and treatment interventions, and decides on a certain course of action. Fellowship Housing’s Outreach Services include:
Referral Process: Fellowship Housing staff complete a Progress Notes for each service provided. Daily communication between Fellowship Housing and Riverbend clinical staff allows both teams to identify progress made, respond more readily to emerging issues and/or concerns, and track availability for increasing outreach capacity.
