Outreach Services for People Needing Supportive Services

Fellowship Housing contracts with the local mental health center to provide medically necessary Outreach Services to designated clients living in the community.  Our staff members provide rehabilitative services in natural community environments, helping to reinforce and enhance the client’s ability to successfully manage the symptoms of mental illness and achieve their personal and treatment goals.  The intent is to help the client avoid hospitalization and gain confidence throughout their recovery.

Outreach Services are based on the goals and objectives of the client's Individual Service Plan.  The services are face-to-face interactions in which our staff members engage the client by building a relationship, using clinical judgment and offering support to help complete everyday tasks and manage symptoms.  Regular communication between Fellowship Housing and the mental health center allows both organizations to best serve the client and address emerging issues.  Fellowship Housing's Operations Director reviews the completed Outreach Services Application to ensure that available services will match a client's needs.

The Outreach Services offered include:

Medication Support:

  • Support, education, supervision and encouragement to clients who are unable to take medication on their own. This service provides assistance to clients around managing the demands of community living.  The goal is for clients to attain full independence and be able to take medications as prescribed with no supervision or intervention.

Therapeutic Behavioral Service:

  • Support in coping with and alleviating symptoms interfering with the ability to successfully function in the community.  This service includes discussions with a client and their supportive parties in an effort to increase the client’s self-management skills such as activities of daily living, household budgeting, etc.

Crisis Management:

  • Direct assistance to a client that helps identify and manage a current or potential crisis situation effectively.
